"When you have a disease with a huge range of severity across a population, focusing policy on average impacts is severely limited in its intelligence.

Swathes of severity are ignored. Inequity multiplies. Progress impeded. Complexity blurred. Risks for vulnerable explode."

Really excellent interview by @erictopol with my risk measurement hero or the pandemic. With transcript.

"from medical school, from all my education, we're not trained to think that viruses, especially respiratory viruses, have these myriad effects and all these organ systems."

Reading @covid_stats (every few minutes), I get the impression that we're all on a honeymoon with (the moment when the virus gives us peace).
One observation: this is exactly the time when a new variant can appear, with less competition.
Welcome to and the sub-variants already detected :covid19:

COVID patients exhale up to 1,000 copies of virus per minute during first eight days of symptoms.

The amount of virus being exhaled while infected was the same no matter which variant a person was infected with — people infected with Alpha exhaled just as much as those infected with , the study reports.

Vaccinated and unvaccinated patients exhale similar levels of virus over the course of infection.

1. viral clearance has never been demonstrated with SARS-CoV-2
2. evidence of asymptomatic persistence has been demonstrated both in vivo and post-mortem
3. persistence is the rule, not the exception
4. reservoirs enable a subclinical form of long covid, which progresses toward a clinical form through various mechanisms, including the exhaustion of T cells and the reseeding of reservoirs upon reinfection

After having simply stated the fact that levels were rising as per last week's update, for today's update the BCCDC saw it fit to massage our interpretation wastewater data, with a concomitant fall in SARS-CoV-2 readings. Natural variance or doctoring? Unsure.

There are signs the latest booster might only be made available to groups that are deemed high risk by our CDC.

The advisory committee is meeting Sept. 12 to decide what recommendations to make. Public comments posted by Sept. 8 will be considered in this discussion.

You can post comments here: regulations.gov/commenton/CDC-

If we want to think about the risk from I think it's very important to keep two scenarios in mind.

We have seen emerge with high variation from prior ancestors

1. that did not take off
2. that exploded

in the population. Both scenarios have happened.

The recent lab studies suggest (1) is more likely than (2). However, we don't have a great track record yet of predicting which variants will (or won't) take off.

So be careful sighing with relief over the recent "good news, it's tame" reporting. It may be right, it may not be.

We won't *know* which of these scenarios are materializing until we see waste water surveillance & hospitalization data. We can watch various bellweather countries & states for early indications.

If you are , then it's time to get out your respirators again and reactivate your airborne precautions.

Of course, some of us have never stopped with the airborne precautions, because we can't risk making the impact we are living every day any worse.

The CDC is taking comments on a National Wastewater Surveillance System for SARSVOV2.

I think some people here might have a comment about that.

"However, the updated booster is not intended for everyone. It’s for people in high-risk groups – those who are age 75 or older, pregnant or immunocompromised, said Offit, who serves on the FDA’s expert panel that reviewed the vaccine companies’ data.

[...] The general public should not expect to need to receive the latest COVID booster, Offit said."
