Basic lack of understanding about the difference between redundant systems (and contingencies) and duplication.
I agree in health care there is a lot of duplication (safety clutter) that doesn’t add to safety or create redundancy but often detracts from it.
However, safety is *not* waste.

@Anaes_Journal 19/ Size / shape coding. The classic #humanfactors engineering intervention by Chapanis involved shape coding of flaps/gear on aircraft. We do it with the O2 knob on the anaesthetic machine we should do the same with drugs.
I have a standard size - 5mls *only* for NMDAs/reversal

Ask junior doctors if they’ve been called to a code to a clinical area and gone to the wrong place or don’t know where it is - over 90% will say yes.
