New adjacent question. I traditionally with opioids start low and titrate up based on response. Has anyone ever seen practice of using weight +\- age to estimate the initial starting dose say of oral morphine? Seen recently (not in palliative care) and was a bit suprised

A question for

We generally avoid putting dexamethasone in 24hr syringe drivers/pumps as we worry about messing with sleep etc and instead give it in the morning and sometimes at lunch (examples of uses are for oedema with brain mets).
But dex has such a long half-life (often quoted at over 24hrs) is there enough clearance on a stat dose to warrant not giving it in a csci just for practicality
