* The COVID-19 pandemic demonstrated the urgent need for expansion ,
NOT REDUCTION, of the Medicaid health insurance program.
* In too many states, political decisions by state legislators to
deny health insurance to thousands of their citizens has resulted in
an almost non-existent social and health safety net.
* Decisions in 2016/2017 by 25 states to reject the expansion of
Medicaid coverage under the Affordable Care Act resulted in an
estimated 7,115 and 17,104 more deaths than had all states opted in,
according to researchers at Harvard Medical School and the City
University of New York.
* The researchers found that because of the states’ “opting out” of
the Medicaid expansion, 7.78 million people who would have gained
coverage remained uninsured.
* To worsen the situation, many states have over the last decade
privatized administration of their Medicaid program into a managed
care program administered by the private profit health insurance
industry.
* The companies are paid by state government, and their profit depends
on spending as little as possible on Medicaid patients.
* It’s hard to imagine any greater disconnect between public good
and private profit: the interest of private health insurance
companies lies not in the obvious social good of delivering quality
health care to patients but in having as few as possible treated as
cheaply as possible.
* No better example exists of a private capitalist enterprise that
feeds on the misery of man.
* You might think that we learned the lesson of discredited managed
care in the 1990s.
* The term “managed care” is confusing to many, but really amounts
to managed reimbursement rather than managed care.
* A set prospective annual payment is made by federal/state
governments, as in the case of state Medicaid managed care, to cover
whatever services patients will receive over the coming year.
* There is therefore a built-in incentive for managed care
organizations to skimp on care and pocket more profits.

Medicare spending has leveled off over more than a decade. One reason why: The Affordable Care Act reduced Medicare’s payments to hospitals and health insurers, reducing per-person spending.
Unfinished business now is finding a way to add dental/vision/hearing benefits to original Medicare. Could they be added to Part B Supplemental Plans or established as a new Part E? People shouldn't be pressured into private Medicare "Advantage" Plans.

The biggest priorities for U.S. health care are closing the Medicaid coverage gap in the ACA-holdout red states and adding dental/vision/hearing benefits to original Medicare.
Unfortunately, it will take progressive supermajorities in both houses of Congress to pass something like single-payer.
It is great for Dems to tout Medicare being able to negotiate drug prices, but let’s hear about what more progress they would make if they win.

Fuck it, now it's a toplevel post.
I hate the Affordable Care Act. It and the employer based coverage system it reinforces has screwed me personally at every turn. My preference for Medicare for All is not about purity as many Democrats may have you believe.
Every year that the individual mandate penalty was there, I was hit with it.

Did you know that under the current form of the ACA nobody need pay more than about 10% of their Adjusted Gross Income for a Silver health insurance plan?
In the rest of the world, people typically pay between about 8-12% of income, so this is pretty great for the US.
It's not great that this has been done by subsidizing for-profit insurance companies and their outsize premiums that in turn fund the outsize bills from so many providers.
But still .. a win!

Health Policy idea:
Add to the Bronze/Silver/Gold/Platinum tier ACA plans a new “Diamond” Tier plan that has no deductibles or copays, covers 100% of the bill, and includes dental/vision/hearing benefits. Premiums would be fully subsidized for anyone up to the 138% of the federal poverty line essentially folding Medicaid insurance into the ACA and closing the Medicaid gap in the red states.
Rebrand the #ACA as “AmeriCare” so that it sounds unpatriotic to try and repeal it. #AmeriCare
Change my mind: the US healthcare "system" is actually a make-work program for rich people's kids who have no desire to run businesses.
The #ACA was an technocrat's iron-clad dream policy. It would work! After all, it "aligned the incentives" of healthcare investors and "harnessed markets" to drive efficiency. No one could accuse this policy - which was copypasted from former Michigan governor #MittRomney's #RomneyCare - of being "socialist." It was invented by a #BainCapital consultant!
