¿APENDICITIS?

#APENDICITIS: MITOS Y VERDADES PARA ESTUDIANTES Y EGRESADOS DE CARRERAS ECONÓMICAMENTE DESESTRUCTURADAS.

Cuando iba a la carrera hubo al menos 5 clases y/o seminarios idénticos: un profesor que era cirujano general nos contaba una caso clínico como el que he visto en esta guardia.

Para sorpresa de nadie el diagnóstico era apendicitis y se hablaba del abordaje del abdomen agudo.

Al final nadie nos hablaba del diagnóstico diferencial del dolor de la fosa ilíaca derecha o nos explicaba la semiología de la apendicitis o su exploración.

Para seguir con el ejemplo vamos a usar la escala de #Alvarado. Buscadla, guardadla, tenedla a mano en la guardia; os va a sacar de apuros y decisiones incómodas.

La escala de Alvarado se usa para orientar diagnóstico clínico de apendicitis.

NO para indicación quirúrgica, pronóstico o idoneidad de prueba de imagen o lo que sea.

#Alvarado story 2; I look out and see a thick black column of smoke rising from their home and run over in alarm. As I am crossing the street I see that it is not the house but in back that the smoke is emanating. Going through the gate, I find all three of them, Tim 13, Sam 12, and Aaron 10 have piled up a huge stack of stuffed animals on the bbq and lit them on fire with impressive results. They decided en masse they had outgrown them. Perils of having #latchkey kids.

1. Across the street I heard Tim and Sam, the older brothers yelling, "Push it again Aaron. Again ..." Looking out I could see Aaron on the porch pushing their doorbell so I go over to see wazzup. I am about 12 and they are 10, 9, and 7; the #Alvarado family. Tim and Sam have used an exacto blade to #disembowl their #goldfish and are attempting to #rescusitate it using wires jumped off the doorbell. Seriously. Living across from them was always entertaining.
