Hey and docs, give me a potential scenario in which in an active battlespace could be useful. Like the patient can't be moved for a while because of enemy fire, and they have a serious injury, how could telehealth be effective and for what?

how do we tackle the first 10 minutes when we know we have a sick brain on our hands, but we don’t yet know exactly what kind of sick brain we’re dealing with?

Need to see if I can borrow these on an inter-library loan. Very interesting reading. I remember comments about if someone has “raccoon sign” (brushing from facial fracture front of skull) that inserting an NG tube can be dangerous - could penetrate the brain if the skull is fractured.

With so much targeted violence now having knowledge a little above first aid could help save someone’s life someday.

Folks in the ED — when assessing a patient with suspected non-epileptiform events (pseudoseizures), it is never appropriate to apply noxious stimuli as a test. Do not nipple twist, sternal rub, compress nail beds, etc.. If you are concerned about the airway, perform a jaw thrust. If you are concerned about seizures, treat as such. If the exam is confusing enough that you can’t be sure, get an EEG (which means admit most places).

as a failed paradigm
Last , 02 am,as a single coverage ED (15 patients waiting to be seen, more than 15 in charge),I see a man with arms pain from 1 month and a women with abdo pain from 2 months.
I remembered of this great talk:we all choose to treat the really sicks when they hit our door.We have to see everyone, but the not-sick-at-all is becoming the standard.
Is emergency medicine a failed paradigm?
