This review gives insight into recent and developing approaches to the management of esophageal cancer, a malignancy that is especially difficult to treat. The authors indicate that the advent of immune-checkpoint inhibitors, which harness and enhance the host immune response against cancer, has substantially improved survival in squamous cell carcinoma, although less so for adenocarcinoma.

Radio frequency ablation (RFA) has become a standard of care for persons with advanced dysplasia in Barrett's, but little is known regarding the occurrence and predictors of post-procedure symptoms. The study found that RFA treatment caused considerable post-procedural symptoms of significant duration in the vast majority of patients, with almost all patients experiencing pain and dysphagia after the procedure.

China has some of the highest incidence rates of esophageal squamouse cell carcinoma in the world. This nice cohort study estimated progression rates to advanced neoplasia (high grade dysplasia or cancer) among those with lower grades, finding that about 1% progressed per year.

While not exactly a streetfight, Drs. Spechler and El-Serag briefly discuss in this "point/counterpoint" possible reasons that current screening approaches for esophageal adenocarcinoma and Barrett's esophagus have had only modest success in affecting incidence and mortality for this cancer.

A quick #introduction. I live in Seattle WA and am Professor of #Epidemiology at the Fred Hutch Cancer Center and Univ Washington.
I love learning new things ranging from web development to #R and #Shiny programming, and have used these tools to develop a website devoted to #esophageal #cancer #prevention. This includes an interactive #risk #calculator and a blog reviewing recent literature.
I have a separate Mastodon account (@tlvee) for my interests in #photography and #painting.

It looks like the incidence of #esophageal #cancer (adenocarcinoma) may be increasing again in white males after a leveling off in the early 2000's. Since the 1970's this cancer has increased about 10-fold in the high-risk group, which is white males.
