AAAS: "Case studies expose deadly risk of mpox to people with untreated HIV" Mpox, formerly known as monkeypox, has plummeted globally since last summer, but still simmers in Latin America and + other areas. In particular, people with unidentified HIV infection who have CD4 lymphocyte counts below 200 — helper-inducer cells in the immune system — are at significant risk of atypical, aggressive infection with mpox. These unusual infections have larger, necrotizing [tissue-destroying] skin lesions + also manifest spread to the lungs. Researchers suggest severe mpox should be added to the qualifying list for a formal AIDS diagnosis. When I started taking care of patients in 1983 in California, there were some 20 sentinel indicators, such as Pneumocyctis pneumonia + cryptococcal meningitis. The list has expanded to at least 24, + aggressive mpox would be the 25th component. "But [a] Lancet report also contained a warning for physicians: Starting coinfected patients on antiretrovirals for their HIV infection could worsen their mpox." About one-quarter of them developed a condition known as 'immune reconstitution inflammatory syndrome,' as the recovering immune system responds to the opportunistic infection with an overzealous inflammatory response. Of this subgroup, more than half died. One more reason to identify + test people at risk for HIV, + persuade them to start antiretroviral treatment. #monkeypox #hivawareness
