Finally after a lot of delay the OCAT trial - Intesification of Radiation in Oral Cavity cancers has been published. Although I am a co-author of the trial involved primarily in the analysis - this decade long clinical trial has some interesting lessons for clinical trialists working out of LMICs. Important @IJROBP Brief Report on the potential for increased mucosal toxicity with proton therapy due to increased end of range relative biologic effectiveness (RBE) and subsequent dose enhancement mirrored by higher PET uptake in regions of injury. Need to examine this closely and make this better if we want to use protons for head and neck cancer. We cannot improve if we don't talk about it! #hncsm #radonc
