Regarding that second picture:
Based on the writing on her shirt, the image itself seems to be a mirror image of actual photo, a copy of which can be seen here: https://wayback.archive-it.org/org-350/20160901005324/https://www.nlm.nih.gov/exhibition/aframsurgeons/morenotable.html
It has no caption here.

Caption calls it an "undated photo" and adds a comment that Bath's “personal best moment” was when she "used an implant procedure to restore the sight of a North African woman who had been blind for 30 years." Photo is credited to her daugher Eraka Bath.

For people who don't put much effort into their ICs, you're missing out on a valuable tool.
Four images gives you 6000 characters to play with to get the stuff that didn't fit into your toot.
Just *always* describe what's in the image *first* so the original intent isn't corrupted. One paragraph to do that, the rest is yours to pack in those #maker technical details.
We *need* better #ImageCaption support for *everybody* not just those *using* readers.

To get a good idea of what mastodon experience is like for screen reader users, you can try using text-only interface to mastodon. Naturally, you cannot see images in your timeline, only alt-text.
I was playing with beautiful Python client called toot - you can just pip install toot, then toot login and toot tui.
