The flora and fauna are really weird. There are some normal, earthly things. But there are also a lot of genuinely unique things. What's this weird thing with glowing things growing out of its head? I have no idea and it's terrifying!

Enemies interact with each other too. You might randomly come across a bandit in the middle of a fight with a hyena. You can use this to your advantage.

Magic and alchemy are available to anyone, but at a cost/investment. Enemies might be resistant or vulnerable to different types of weapons or spells, but you don't know which until you try them. You have to go to specific places to learn magic from NPCs.

Shops don't have everything all the time, not do they have unlimited inventory. You might have to travel a significant distance to find something you're looking for.

You have to sleep. You have to have some gear to sleep out in the wild. If you sleep out in the wild, you have take precautions or you will be ambushed while asleep.

Travel takes time. Healing takes time. Asking a blacksmith to craft high level gear takes time. Equipment wears out and breaks. Natural resources grow back, eventually, but you have to wait.

Realistic encumbrance, along with heat/cold, hungry/thirsty, and diseases. Torches and lanterns burn out! You better bring enough supplies!

You want a map? You make it. Indoor areas have no maps whatsoever; outdoor areas have a high level static map that does show your position. To get around you have to navigate by landmarks.

Focus on loot and silver. There is literally no XP. You are risking your life to go into dungeons and wilderness to get money and better stuff.

High lethality, combined with persistent world combined with no rewind to previous save points. (Not perma-death though.) As you explore and die, you develop a history and a narrative regarding the various encounters. That bandit that killed you? You can go back and kill him later for revenge.

There are no classes, you are defined by what you carry and skills you know. You have to actually find and pay a trainer to teach you any given skill, you don't just get them "for free" by levelling.
