Travel takes time. Healing takes time. Asking a blacksmith to craft high level gear takes time. Equipment wears out and breaks. Natural resources grow back, eventually, but you have to wait.

You want a map? You make it. Indoor areas have no maps whatsoever; outdoor areas have a high level static map that does show your position. To get around you have to navigate by landmarks.

High lethality, combined with persistent world combined with no rewind to previous save points. (Not perma-death though.) As you explore and die, you develop a history and a narrative regarding the various encounters. That bandit that killed you? You can go back and kill him later for revenge.
