Couple of reasons (from a few days experience):
1. #SensitiveContent (just getting the obvious one out of the way đ )
2. server load - when it's heavy I see either a #PlaceHolder or blurred image.
Someone may be able to confirm if a server auto degrades images in heavy load situations.
