"Cancel Service" is located in a reasonably placed section of the account home after logging in.

Clicking the button leads to a screen asking whether to cancel some or all services.

Ticking "all" reveals a set of selections to indicate the reason. Options include: moving, another provider for speed, another provider for price, financial hardship, military, deceased, disaster, and other.
