When there is bilateral sacroiliitis with erosions, DDx include:
1.hyperPTH in setting ESRD
2.ankylosing spondylitis
3.IBD
4.RA
In this case, look at the cortical resorption and the osteopenia in other areas. The pt has hyperPTH related to ESRD- Dr. Avneesh Chhabra

Anasarca vs myositis?
DIFFUSE edema is seen in anasarca and PATCHY edema is in myositis. In this case, the patient has both. On MRI with fat suppression, you can see the fluid as bright. – Dr. A Chhabra
