One of the problems with is that it implies there is always a root cause that can be resolved, or that the root cause isn't implemented on purpose by bad decision making. I work in an environment in which at least half of ongoing user issues are the result of bad technical implementations and decisions, not malfunctioning tech. It's working like it's supposed to, it's just not user friendly.

The US Chemical Safety Board (CSB) is "an independent federal agency charged with investigating industrial chemical accidents."
