Compelled to note a recent recall from Nissan for effectively the same thing - except, it was not a defect from the factory, and Nissan issued a voluntary recall after finding 2 incidents during their own dealer audit.
Contrast to Tesla: an issue at the factory (with all the QA/QC that should be taking place at a factory), no voluntary recall from Tesla after 2 incidents *reported by owners while driving*, & NHTSA has to launch an investigation.

In case anyone doubts that #safetyculture actually works...
I put on nitrile gloves for my battery swap (since the engine compartment is filthy). While securing it in place, I noticed one of the gloves had ripped. Aside from swearing (one of many instances), my immediate thought was "I should report this". Nevermind that it's been a year or more since I needed to enter a lab area. It took me a moment to realize I was at home and in my driveway.
