Indeed. I think it is one thing to say that at a system level that we need to expect (or accept?) lower standards of care quality, and another thing entirely to see how that holds up when scrutinised at an individual incident level after the fact.

Interesting description of how the Calgary region instituted a 'just culture' following a pharmacy error.

Would recommend for those interested in system safety, healthcare safety and safety science.

Also, my lovely wee pup seemed to like it too 😊
