Post-Harry Potter era (around 100 years later or so) where new death eaters are trying to revive Voldemort's ideals. The young followers are big on revelries, where one of the "main events" is deflowering a virgin on an altar. The spectators are the death eaters themselves and their followers.
These pure-blooded virgin sacrifices are offered to the Dark Lord, whom they're convinced will one day reincarnate.

Queen Yuuji doesn't realize that he's queen.
Itadori Yuuji thought he was kept by Sukuna's side as both hostage and ambassador for humankind but didn't realize that he was actually crowned queen consort. It's just that nobody ever called him "Queen."
Only "Your Highness" or "Yuuji-sama" for Uraume. Especially when he sees that Sukuna has accumulated a harem for himself when Yuuji was in a (5 years? 10?) coma after their separation.
