Following harmful healthcare incidents (we are talking deaths here!), the need to be sensitive to timing was something that came across very strongly. For some people, investigations moved too quickly and didn't allow for grieving or coming to terms with what happened. For others, delays prevented people from moving on and contributed to compounded harm. The need to respond meaningfully within the current system is being debated.
