Our literature review did suggest that psychosis was more prevalent in the transgender community as compared to general population prevalence estimates and cisgender comparison samples. In our paper, we argued for a more critical examination of why rates of psychosis are elevated. Specifically, we highlighted the potential for diagnostic biases (ie., that transgender people are misdiagnosed with psychosis) and the fact that transgender people face high rates of known social and environmental contributors to psychotic disorders (e.g., bullying and discrimination). We suggest that these factors may increase transgender people's vulnerability to psychosis and other forms of psychological distress.
Rather than suggesting that "unaddressed psychiatric issues" (such as psychosis) could be "misdiagnos[ed as] gender dysphoria" and lead to "inappropriate" gender transition, our claims are that 1) psychological distress due to gender dysphoria and oppression- and marginalization- based risk factors increase the likelihood of serious mental health symptoms, like psychosis, among transgender people, and 2) historical diagnostic trends and biased conceptualizations of gender dysphoria suggest the possibility that some transgender individuals are misdiagnosed as being psychotic.
