The use of upper case and underscores in hashtags is more important to other disabities like #dyslexia (or even without disabilities at all) and if you see/read the text. If you serve them you almost automatically serve #ScreenReaders as well.
The other way around, most screen readers pronounce most hashtags very understandable even if they are completely in lower cases, but even those could still not be accessible for people with dyslexia.
